Paperwork needed for all Courses:

1.  Medical History Form - Dr Signature may be required prior to course starting.
2.  Responsible Diver Code
3.  Assumption of Risk and Liability 
4.  Youth Addendum - for all divers under the age of 18

Other Flyers:

1.  Sat, Dec 6th - Chipotle Fundraiser - 9940 W Happy Valley Road 4 pm - 8 pm.  25% of proceeds go towards our scholarship fund to ensure that divers who need adaptive training or specialized equipment — but may not have the financial means — can still experience the life-changing freedom of scuba.
